By submitting inquiry for services with Colwellness ("Company"), the undersigned participant acknowledges and agrees to the following release of liability:

  1. Assumption of Risk: The participant understands and acknowledges that the training sessions may involve strenuous physical activity, including but not limited to, strength training, cardiovascular exercises, and flexibility training. The participant voluntarily assumes all risks associated with such activities, including the risk of injury, illness, or death.

  2. Physical Condition: The participant affirms that they are in good physical condition and have no medical conditions that would prevent their participation in the training sessions. It is the participant's responsibility to inform the Company of any medical conditions, injuries, or physical limitations prior to engaging in services.

  3. Release of Liability: In consideration of being allowed to participate in the services, the participant, on behalf of themselves, their heirs, assigns, and next of kin, hereby releases, waives, discharges, and covenants not to sue the Company, their employees, agents, and representatives from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant during or arising from services provided, whether caused by the negligence of the Company or otherwise.

  4. Indemnification: The participant agrees to indemnify and hold harmless the Company, their employees, agents, and representatives from any and all liabilities, claims, demands, actions, or causes of action, including reasonable attorney's fees, arising out of or related to the participant's participation in services provided.

  5. Emergency Medical Treatment: In the event of an injury or medical emergency during service, the participant authorizes the Company to obtain emergency medical treatment for them as deemed necessary. The participant agrees to be responsible for any costs associated with such treatment.

  6. Severability: If any provision of this release of liability clause is found to be invalid or unenforceable, the remaining provisions shall continue to be binding and enforceable to the fullest extent permitted by law.
     

By submitting your information to Company, you understand the terms of this release of liability clause, and voluntarily agree to be bound by its terms.
